California: Controversy Over Taxpayer-Funded Gender Transition Care for Undocumented Immigrants

Claims have emerged that undocumented immigrants in California are receiving gender transition surgery and hormone therapy through state-funded medical programs. Chris Rufo, a fellow at the Manhattan Institute, filmed encounters and released footage of transgender undocumented immigrants outside taxpayer-funded homeless shelters in San Francisco. Rufo alleges that some undocumented immigrants are accessing gender transition-related procedures through Medi-Cal, California's healthcare program for low-income residents.

According to the California Department of Health Care Services (DHCS), the state is spending approximately $9 billion in taxpayer money on healthcare for undocumented immigrants, including gender-affirming care. Approximately 1.7 million undocumented immigrants have received full Medi-Cal coverage.

In response, DHCS refuted the claims, stating that Rufo's footage and reporting contain significant factual errors and distortions regarding Medi-Cal eligibility and coverage. "It is categorically false to suggest that Medi-Cal broadly covers gender confirmation surgery without limitations for any group," DHCS stated. "Medi-Cal is a needs-based program with strict eligibility requirements." The department explained that coverage for gender-affirming care is applied only when it is considered 'medically necessary' according to clinical standards and nationally recognized clinical guidelines.
